Previous prescriptions and the medicines you take now

Any scans you have: MRI, CT or X-ray films and their reports

Laboratory reports, however old

Earlier EEG or nerve conduction reports

Discharge summaries from any hospital stay

A relative who knows the history, if the problem involves memory or fits

Who is seen at the clinic

Adults with a neurological problem

Headache, fits, stroke, memory problems, numbness, tremor, and disorders of the nerve and muscle.

Children

Child neurology is seen at the clinic. Bring earlier records and, where you can, someone who knows the history.

General medical patients

Dr. Umar Zafar Ali is also a General Medical Specialist, so general medical problems are seen as well.

Second opinions and reviews

Patients already on treatment who want the diagnosis reviewed, or who have not improved elsewhere.
